PRIMARY CARE NURSE

- m8 I! s3 }$ ^1 E6 G) vThe Primary Care Nurse will provide comprehensive nursing services to the patient population of a family practice clinic within the Network. The Nurse will assess, manage and support patients, providing appropriate documentation, and will work collaboratively with the physician and multi-disciplinary team to develop and implement care plans and to provide follow-up with the patient. The Nurse will work to full scope of practice within the primary care setting; some duties will vary depending on the clinic and patient population.

• To receive patients and provide individualized health care and services based on highly developed nursing knowledge and skills.

• To work in conjunction with the physicians or independently as appropriate to incorporate the nursing process to provide patient centered, goal oriented care.

• To assist in the delivery of disease management and health promotion programs according to clinical practice guidelines.! t ?0 k3 C0 j: r4 D' v+ M: o
• Ensure the maintenance of accurate records in line with practice policy.

- ^( }5 n) Z4 |+ s# M• Baccalaureate degree in Nursing preferred
5 x1 P0 L3 U2 n3 ]/ r5 F7 T• Preference given to those with at least 5 years nursing experience in primary care setting.6 H0 O0 V3 `+ t$ [6 B; ~% i. B
• Current CARNA Registration5 Z6 P% G9 m" ]$ O
• Current CPR – Healthcare Provider
2 A: @6 [3 }$ }* f3 i) E0 N* E5 ~• Must have own vehicle and valid driver’s license
8 ?2 i1 |, L3 O2 L+ ?• Knowledge of, and ability to use, electronic medical records
